David's Fees *
$175 for the initial session
$150 for each subsequent session

If using insurance, this is the amount that will be billed to your plan. Your responsibility will be your plan's copay, coinsurance, or deductible amount.

*Ridgeline Counseling shall disclose, in advance, all fees, including but limited to any charges for late cancellations or missed appointments at the beginning of treatment and give reasonable notice of any changes in fees or other charges. Sliding fee scales are available for those with no insurance coverage or who may opt to self-pay. For rates regarding additional services, including the sliding fee scale offered by David Markwell, please refer to the documents listed under David's Forms . Other therapists may also have sliding fee scales available upon request.

Insurance FAQ'S

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health benefits?
  • Is authorization required? (If yes, please obtain an authorization number)
  • What is my deductible and has it been met?
  • How many sessions per calendar year does my plan cover?
  • How much is my copay?
  • How much does my plan cover for an out-of-network provider?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
Common Insurance Billing Terms: Procedure/CPT Code : a billing code sent to your insurance company describing the type of service performed. Charge/Billed Amount : the amount we bill to the insurance company for services performed. Reasonable and Customary Rate : The rate agreed upon between our office and your insurance company. Adjustment : The difference between the charge/billed amount and the reasonable and customary rate set by insurance company. This amount is written off. Deductible : The amount you must pay out of pocket for services before your insurance will begin to pay. Coinsurance : Out-of-pocket amount split between you and your insurance company. Copay : A fixed amount you pay at each visit.

Payment

Cash, checks, all major credit cards, Health Savings Cards, and Flexible Spending cards are accepted for payment. Payment may also be submitted online as described here .

Cancellation Policy

There is a no show/late cancellation fee charged for clients who do not show up for the scheduled appointment and have not notified the office at least 24 hours in advance.
